In order to establish terms with Burdge for ongoing business we require that you complete the enclosed Credit Application.pdf by typing directly to the form, then print and sign & fax it, Attention to Accounting Dept 1-323-887-7795. If your credit application is approved, future orders may then be purchased on a net 30 basis.
If you will be RESELLING any products you are asking us to print, you will also need to completely fill in, sign, and fax back to Joe Lee, accounting manager, at FAX 1-323-887-7795 the California Resale.pdf form at the time your order is placed. Without this form on file at Burdge, we are required by law to charge any applicable sales taxes based on where we ship your final product.
